Holy Childhood’s Community Habilitation Program provides families and individuals who live in the community with essential life skills training so that they can develop the ability to live and work independently. Our staff support people 1:1 or within small groups while assisting them in the areas such as accessing community resources, transportation training, daily living skills and social skills training.
Community Habilitation
Community Habilitation services are supports that are provided to participants individually or within small groups. These supports can be delivered at any non-certified location, including the person’s home. Community Habilitation services primarily deliver services that facilitate community inclusion, promote independence, and nurture person centered goals.
Note: For all referrals – please remit current copies of the following required information to nzelesnikar@holychildhood.org
Insurance Cards – Medicaid Benefit Card and all other insurances the person is covered by.
Most recent psychological evaluation and social evaluation (MSC).
Current Life Plan (or addendum done by Care Coordinator) listing “Article 16 Clinic, Holy Childhood” to providers section required on approval before start of services.
